Home Care

There's no place like home. For many patients, that's just what the doctor may order for a speedy recovery. Home health care, provided by Jefferson Health Home Care and Hospice, is a service for persons who are too sick or disabled to care for themselves, but well enough not to be admitted to a hospital or nursing home. Studies indicate that patients who stay at home, or return home to recover, may get better more quickly.

Compare Medicare-certified home health agencies on Medicare’s home health compare website – Our Home Care Services have scored better than other local agencies on helping patients to get better and improving patient outcomes. Our Home Care services have also consistently scored above the state and national average for patient outcomes and patient satisfaction on most of the indicators.

Our Home Care Department is widely recognized for the quality and scope of its service.  Its goal is to preserve and improve each patient's life at home by emphasizing independence and self-involvement in the care process. Home Care is certified by Medicare and is accredited by The Joint Commission. Home Care and Hospice earned CMS’s 4-star rating.

A Tradition of Caring

Every year, thousands of patients and their families in the Delaware Valley turn to us for compassionate, skilled nursing and other care in the comfort and security of home. Patients rely on us to provide professional care and sophisticated procedures, once only available in the hospital, in surroundings that are familiar and reassuring.

Commitment and Character

Home Care staff have chosen home care as their profession. They understand the importance of safety, comfort and dignity to patients and their families. They are highly educated, routinely attend classes to keep their knowledge and skills current and subscribe to the highest professional standards, delivering care with sensitivity to the patient's desire for privacy and independence.

Comprehensive, Cost-effective Care

As an alternative to costly hospitalization and lengthy stays, when patients are discharged, they may choose a home health care provider. Our Home Care Department offers a comprehensive range of services on site.

Working with physicians, case managers and family members, our professional staff determines the clinical and personal care plan that best meets patients' needs. Under the care of registered nurses, therapists, medical social workers and aides, patients benefit from high-quality, skilled, reliable and efficient care while enjoying the relative independence of home.
