One-Call Transfer

To referring physicians:

At Abington Hospital – Jefferson Health, we believe that transferring patients should be a simple, speedy and safe process. Patient safety is our number-one priority, and as time is usually a major factor, a seamless process is imperative for clinicians needing to transfer a patient.


for One-Call Transfer

With just one call to the AH Transfer Center, 215-481-8181, transferring your patients to AH is quick and easy.

In an effort to expedite and streamline patient access, our program is staffed with registered nurses and is available 24 hours a day, seven days a week.

Materials are available on request to provide you with the essential tools for an efficient transfer. Please note the transfer exceptions include: serious burns, complete spinal cord trauma (quadriplegic or hemiplegic) and pediatric (under the age of 16) trauma.

The simplified process outlined will take you through each step to ensure that your patient’s transfer is expedited and that care is well coordinated. We recognize that communication about your patient is important and we pledge to provide updated information as soon as it is available.

5 Easy Steps

Step 1: Call 215-481-8181

Step 2: Provide clinical information:

  • Patient name
  • Patient age
  • Diagnosis
  • History
  • Medications
  • Assessment
  • Vital signs
  • GCS trauma/neuro
  • Interventions
  • Test results

Step 3: Physician-to-physician communication

Step 4: AH arranges transport (please inform us of any special transportation needs)

Step 5: Prepare patient for transport and include the following:

  • Copy of transfer consent form
  • Copy of patient’s medical history
  • Copy of ancillary testing results
  • Diagnostic studies
