The Hospice Team: People Who Care

Hospice care is a way of helping terminally ill patients live more fully and comfortably at home during the last phase of life.

Abington Memorial Hospital Home Care has a holistic hospice program that is ready to help.

Our team of caregivers includes a medical director, nurses, therapists, home health aides, a chaplain, social workers and volunteers. Care is coordinated through weekly team meetings in cooperation with family physicians.

Concern for Everyone Involved

A special aspect of hospice care is concern for all members of the family, which differentiates this type of care from other traditional forms of health care. Although the physical care and comfort of patients are our primary concern, the hospice team also focuses on supporting family members, both emotionally and spiritually. Bereavement support is available to help family and friends cope with their loss.

Eligibility for the Hospice Program

A person suffering from an illness for which palliative care (treatment without curing) is most appropriate is eligible for hospice care.

The individual also must wish to remain at home during the last days of life. It is vital that family and physician support the patient's decision, and that someone (either a family member or friend) is prepared to accept primary responsibility for the patient's care.

Patients can be referred to our hospice program by their doctors, the hospital, another agency, family, members or directly by themselves.

Insurance Coverage

The costs of hospice care may be reimbursed under Medicare, Blue Cross, Medicaid, or other third-party payers. Many private insurance companies offer special hospice benefits. Abington Memorial Hospital Home Care is a Medicare-certified Hospice and is accredited by the Joint Commission. Service is available to all persons, regardless of race, color, sex, religion, age, national origin, handicap or ability to pay.
