Home Infusion Therapy

Coping with a medical condition that requires medication to be infused directly into a vein can be challenging. Providing infusion treatment in the comfort of home enables patients to return to familiar activities, take an active role in their care and even, when appropriate, return to work.  Returning to the familiar activities of life offers other emotional benefits and can facilitate healing. In addition, home infusion is a cost-effective approach to receiving IV therapy. Abington Health’s high-tech infusion services assist patients in meeting short and long-term infusion therapy needs in their own environment.

The Home Infusion Team

Abington Health’s Home Infusion program’s multi-disciplinary team, led by a doctor of pharmacy, consists of pharmacists, infusion specialists, registered nurses and registered dieticians. Infusion staff is IV- and PICC-certified and serves patients ages 12 and older.  Care is provided 24 hours a day, seven days a week.

Under the direction of a patient’s physician, the team coordinates the provision of medications, supplies and equipment and facilitates delivery to the home. A patient’s response to therapy is monitored by staff and results and recommendations are communicated to the patient’s physician.

Quality Care

In providing compassionate, quality care with a focus on patient outcomes, the Home Infusion team takes the time to provide patients and caregivers education specific to their medications, equipment and prescribed therapy. They can also coordinate additional home care support services, including medical/surgical/palliative nursing, physical therapy, occupational therapy, speech therapy, social work or psychiatric care. Along with scheduled visits, Home Infusion staff is available by phone 24 hours a day, seven days a week to address questions or concerns. Abington Health’s Home Infusion program is Joint Commission accredited.
